Estate Accounts1,2
Estate accounts are designed to provide depository and payment services to the decedent’s estate. In addition to an account addendum and BSA form that we will provide, you will need a few pieces of documentation, including:
- Completed account application
- Original letter of testamentary
- EIN Number
- Certified copy of a death certificate

Conservator/Guardian Accounts1,2
Conservatorships and Guardianships are legal appointments made by a state court whereby an individual is given authority to act on behalf of a minor or an incompetent adult – referred to as a “ward.” In addition to an account addendum that we will provide, you will need a few pieces of documentation, including:
- Completed account application or account change request form
- Certified copy of conservator order or guardian order
- Identification of conservator/guardian

Revocable and Irrevocable Trust Accounts1,2,3
A trust is a separate and distinct entity created by a person or persons for the purpose of holding certain assets for the benefit of identified persons. Establishing a trust can reduce paperwork and provide other benefits. In addition to an account addendum and BSA form that we will provide, you will need a few pieces of documentation, including:
- Completed account application or account change request form
- Trust Documents

Representative Payee1,2
A representative payee is appointed by the Social Security Administration or the Veterans Administration to manage the funds for someone who cannot handle his or her own financial affairs. In addition to an account addendum that we will provide, you will need a few pieces of documentation, including:
- Completed account application or account change request form
- Appointment of representative form from SSA or the VA
- ID of beneficiary if available
- ID of representative payee

Power of Attorney
Powers of Attorney are legal documents that provide for the delegation of authority from the principal to an agent.
The original or certified copy of the power of attorney documents will need to be mailed or dropped off at one of our branches. The Specialized Accounts Department will mail all other required documents after the power of attorney has been reviewed.
